Waiver of Liability form

By using the camp, all participants agree to the "Boot Lake Camp Policies" which details the camp facilities, code of ethics and expectations of those using the Corporation's property.

IMPORTANT: All groups visiting Boot Lake Camp must submit a signed waiver of liability. Please complete the following form as soon as your reservation has been confirmed by the camp director. Once submitted, you will receive a copy of your waiver via email. If you have any late additions or changes to your previously submitted waiver, please use the link the provided in the email.
